About this Module:
Neuromodulation in Practice: Peripheral Nerve Stimulation (PNS) is a procedural video module featuring three expert-led demonstrations of peripheral nerve stimulation techniques for common pain indications. Faculty provide step-by-step instruction on patient selection, anatomical targeting, imaging guidance, and lead placement strategies for the superior cluneal, sciatic, tibial, and genicular nerves. Through fluoroscopic and ultrasound-guided approaches, learners will review key anatomical landmarks, procedural planning considerations, and techniques to optimize lead placement and safety. Practical insights on device ergonomics, infection control, and post-procedural management are also discussed to support successful integration of PNS into clinical practice.

Following participation in this activity learners will be able to:

  • Describe fluoroscopic identification of the iliac crest and the basic lead placement steps for superior cluneal nerve PNS.
  • Identify appropriate sciatic and tibial nerve PNS targets and procedural approaches based on patient pain distribution, anatomy, and device-wear considerations.
  • Demonstrate fluoroscopic targeting and lead placement techniques for superior genicular nerve PNS based on patient pain distribution and anatomical landmarks.

Presentation 1: Superior Cluneal Nerves
Faculty: Alaa Abd-Elsayed, MD, MPH

Dr. Alaa Abd-Elsayed demonstrates fluoroscopy-guided peripheral nerve stimulation (PNS) lead placement for the superior cluneal nerves. He reviews basic anatomy, diagnostic nerve blocks, fluoroscopic identification of the iliac crest, and step-by-step needle and lead placement. He also discusses post-operative considerations like infection control, lead migration, and patient follow-up. 


Presentation 2: Sciatic & Tibial Nerve Targets
Faculty: Scott G. Pritzlaff, MD

Dr. Scott Pritzlaff reviews peripheral nerve stimulation (PNS) targeting the sciatic and tibial nerves for the management of lower-extremity neuropathic pain. He covers key considerations for patient selection, device ergonomics, and procedural planning. Using a step-by-step approach, Dr. Pritzlaff demonstrates both perpendicular and parallel lead placement techniques for sciatic nerve PNS, emphasizing ultrasound visualization, patient positioning, and risk mitigation. He also reviews tibial nerve PNS indications and technique, including approaches at the popliteal fossa and the tarsal tunnel, with relevant anatomical landmarks and safety considerations.


Presentation 3: Genicular Nerve Targets
Faculty: Johnathan Goree, MD

Dr. Jonathan Goree reviews peripheral nerve stimulation (PNS) targeting genicular nerves for the management of severe knee pain. He discusses patient selection, diagnostic nerve blocks, fluoroscopy-guided targeting, and step-by-step lead placement techniques at the superior medial and superior lateral genicular nerve targets. He also reviews depth considerations, lead positioning along the femoral condyle, manufacturer-dependent implant techniques, and infection control practices.

The documentation should indicate the PA's name, the accredited ACCME organization's name, the CME activity's title and date, and the number of credits designated for Category 1 Credit.  NCCPA provides the option to upload CME certificates when logging in, but you are not required to submit documentation at the time that you log credits. However, be sure to keep documentation of all completed Category 1 CME activities for 7 years in case you are audited by NCCPA.  Check with your state licensing board regarding their individual auditing and documentation requirements.
